General (Non-ED or Inpatient) Workflows

  • Consult Workflow - (Handout) Use the Transfer Center activity to document steps involved with a consult for a patient from an outside facility or referring provider to help determine if they need to transfer to UIHC or not.
  • Neurology Telestroke Workflow for ATC - (Handout) The ATC creates the one-click appointment for the Neurology Telestroke encounters.

Transfer Center Intake Resources

  • Creating an Anonymous Patient during Intake - (Handout) If the ATC receives notification of an expected patient but are not given other details (such as gender, date of birth, name, etc.), then they are able to create an Anonymous Patient in Epic.
  • Creating New Patient Medical Record Numbers (MRNs) - (Handout) New patients need a Medical Record Number (MRN) created if they do not already exist in Epic.
  • New Call Workflows - (Handout) The Transfer Center New Call activity provides the ability to document calls before knowing which request it is meant to be attached to and to search for the intended request after documentation is complete. Additionally, if call details are attached to the incorrect request, you can move call details between requests.
  • Requesting Care Everywhere for Intake - (Handout) To request Care Everywhere during the Intake process (before a pending admission is created) and for ALL Consults, the Admission and Transfer Center can request outside records.
  • Super Triage Voalte Message - (Handout) To provide consistent medical information to Super Triage when sending Voalte messages, a Super Triage Voalte (.stv) smartphrase can be added to the Call Summary field of Contact History, after required navigator fields and smartforms have been completed.
  • Undo a Canceled Intake - (Handout) When an intake is canceled, the associated expected encounter is canceled as well. The cancellation of an intake can be undone,setting it back to the previous status. However, re-opening the intake does not un-cancel the expected encounter. In order to create a new encounter, you must create a new preadmission for the intake.
  • Update a Pending Preadmission: ED to Direct Admit & Direct Admit to ED - (Handout) Update a pending preadmission when the Request Type changes during patient intake (Examples: Direct Admit to ED or vice versa).
  • Update a Pending Preadmission for Location Change - (Handout) If the ATC is planning a Direct Admit to a specific location, then it is determined that the patient will go to another location, use the following process to change the Direct Admit pending preadmission from one location to another.
