Therapy Plans are a pre-defined set of orders that are administered to a patient during multiple encounters at specified intervals, such as for dialysis or infusion therapy. A Therapy Plan is created when a provider takes a base protocol and then modifies it to be patient-specific. Based on information entered by the developer and then by the patient’s provider, the system calculates when particular orders should appear as due or available for action by a clinician.

The following resources provide information on Therapy Plan functionality.

  • Therapy Plan: Day of Treatment (ONC071) - (eLearning: 4 min) Begin treatment, release orders, administer medication and complete the treatment day for therapy plans.

  • Ordering and Managing Therapy Plans (ONC070) - (eLearning: 5.5 min) Find, apply, manage, and sign a therapy plan.

  • Configure the Schedule Activity (for Therapy Plans) - (Handout) Configure the Schedule activity to determine if a plan has been ordered for a patient and view the details of any existing Therapy or Treatment Plan.

  • Dialysis CROWNWeb Abstraction Report - (Handout) A report is now available within Epic to facilitate the sending and receiving of data to and from CROWNWeb.

  • Releasing Orders - (Handout) Physicians select orders from a protocol, modify them as needed so that they are appropriate for the patient, and sign them. Nursing staff will then release the orders each visit in order to make them active. This helps to improve clinic workflow and allows providers to enter orders once to be used for every visit for the course of treatment.

  • Reporting Workbench - (Handout) Expired plans and plans with orders signed by inactive providers are unable to be released and acted upon. In order to assist with this functionality two new reports are now available in Reporting Workbench Reports (RWB) for each therapy plan area. Search ‘Therapy Plan’ in RWB to run and view these reports.

  • Snapboard Scheduling (Dialysis) - (Handout) The Snapboard is a tool used for making appointments or scheduling treatment in areas where patients do not stay overnight. This resource includes how to access and schedule with the Snapboard as well as how to make it a favorite activity and access additional functions via right-click.

  • Specialty Plans of Care - (Handout): Specialty plans of care let everyone involved in a patient's care contribute to the same plan to make sure that they're all in agreement when it comes to the course of treatment. Physicians, nurses, social workers, and dietitians can sign off on their portion of a dialysis plan and make sure that required documentation is completed in a timely manner.

  • Updating Order Diagnosis - (Handout) To update a diagnosis on orders that were previously signed as part of a Therapy Plan, the order must be resigned. The plan and orders do NOT need to be cancelled, removed, or restarted.

  • Working with Plans from In Basket - (Handout) Treatment Plan details and related messages can be sent directly to a user's In Basket to streamline communications and facilitate access and modifications to the plan.
