Inpatient and outpatient dialysis areas at UIHC are given functionality that is at once ambulatory, procedural, and inpatient. Clinic and inpatient staff use similar navigators and a common flowsheet. The inpatient unit utilizes the Snapboard for scheduling treatment (similar to some procedural areas) and sometimes cares for patients on unit and other times travels to their bedside. For that reason, this page was created to centralize their education resources.

  • Dialysis CROWNWeb Abstraction Report - (Handout) A report is now available within Epic to facilitate the sending and receiving of data to and from CROWNWeb.

  • Snapboard Scheduling (Dialysis) - (Handout) The Snapboard is a tool used for making appointments or scheduling treatment in areas where patients do not stay overnight. This resource includes how to access and schedule with the Snapboard as well as how to make it a favorite activity and access additional functions via right-click.

  • Specialty Plans of Care - (Handout): Specialty plans of care let everyone involved in a patient's care contribute to the same plan to make sure that they're all in agreement when it comes to the course of treatment. Physicians, nurses, social workers, and dietitians can sign off on their portion of a dialysis plan and make sure that required documentation is completed in a timely manner.

Clinic Schedule

  • Checking out an Appointment - (Handout) During Check Out, registration information can be updated, copays collected, and follow-up appointments scheduled.

  • Clinical Workflow for Patient Check In - (Handout) Clinical staff may need to check in patient appointments in areas where patients are transported directly for patient care or when there is not a front desk staff available.

  • Configuring a Personal Schedule - (Handout) A personal schedule allows you to track the patients you see each day and quickly access their patient visits. Customize the schedule view of the patient information to display those columns which are most beneficial.

  • Get Up to Speed from the Schedule (POSS082) - (Video: 2 min) Learn how to use the schedule, including how to change your display and personalize your toolbar.

  • Overview of the Schedule Activity - (Handout) Also known as the “Multi-Provider Schedule”, the Schedule provides quick access to daily appointments for a single provider or for multiple providers and resources within a department. From the Schedule, a user can perform actions on the selected patient’s appointment, change the schedule and date that displays, and view basic patient and appointment information.

Therapy Plan

  • Configure the Schedule Activity (for Therapy Plans) - (Handout) Configure the Schedule activity to determine if a plan has been ordered for a patient and view the details of any existing Therapy or Treatment Plan.

  • Ordering and Managing Therapy Plans (ONC070) - (eLearning: 7 min) Find, apply, manage, and sign a therapy plan.

  • Releasing Orders - (Handout) Physicians select orders from a protocol, modify them as needed so that they are appropriate for the patient, and sign them. Nursing staff will then release the orders each visit in order to make them active. This helps to improve clinic workflow and allows providers to enter orders once to be used for every visit for the course of treatment.

  • Reporting Workbench - (Handout) Expired plans and plans with orders signed by inactive providers are unable to be released and acted upon. In order to assist with this functionality two new reports are now available in Reporting Workbench Reports (RWB) for each therapy plan area. Search ‘Therapy Plan’ in RWB to run and view these reports.

  • Therapy Plan Quick Start Guide - (Handout) Indexed Quick Start guide for Providers on how to use the Therapy Plans activity.

  • Updating Order Diagnosis - (Handout) To update a diagnosis on orders that were previously signed as part of a Therapy Plan, the order must be resigned. The plan and orders do NOT need to be cancelled, removed, or restarted.

  • Working with Plans from In Basket - (Handout) Treatment Plan details and related messages can be sent directly to a user's In Basket to streamline communications and facilitate access and modifications to the plan.
