Identity is Epic's ID and duplicate record management application.

Resources

The following resources provide information on Identity functionality.

  • Changing a Patient Name - (Handout) The Demographic form is used to update a patient’s name whether it is due to a name change or if it was entered incorrectly. The user will update the Name Edit window and indicate a reason for the change.
  • Chart Correction - Contact Mover - (Handout) The Contact Mover tool is used to move a contact (encounter) and its associated records from one patient’s chart to another when the patient contact was added in error to an incorrect patient’s chart.
  • Chart Correction - Note Mover - (Handout) The Note Mover tool is mainly used to move a contact record(s) or notes from one patient’s encounter to another when the patient note was added in error to an incorrect encounter.
  • Chart Correction - Patient Merge - (Handout) The Chart Correction Merge process is used to merge patient charts where duplicate patient charts were created in error.
  • Chart Correction - Patient Unmerge - (Handout) The Chart Correction Unmerge tool is used to unmerge patient charts where patient charts were merged in error.
  • Creating New Patient Medical Record Numbers (MRNs) - (Handout) New patients need a Medical Record Number (MRN) created if they do not already exist in Epic. 
  • Marking Patients for Merge - (Handout) When duplicate patients are created in the system by accident, the identity team can merge the two patient records. If a potential duplicate patient is identified on the front end, the patient should be marked for merging so it can be reviewed for a possible merge.
  • Merging a LINK MRN - (Handout) When outside facilities send patient information to UIHC via UI CareLink, a LINK MRN (temporary Medical Record Number) is created which must be merged with a patient’s permanent MRN. The Patient Merge activity is used to merge the LINK MRN with the permanent MRN.
  • Patient Information Help Desk In Basket Messages - (Handout) Select HIM staff will work Help Desk In Basket pool messages with Patient Information as the type. The reporting tool is designed to gather information from reporters to facilitate working with these messages. The HIM user can forward the message to another appropriate user or reply to the sender directly from the In Basket pool. Chart Correction Cases can also be created directly from In Basket.
  • Patient-Level Registration - (Handout) Patient Level Registration is information about the patient that is documented in Registration, whether for an ED visit, admission, or outpatient appointment.
  • Patient Photos - (Handout) A photo of the patient may be stored and viewed within the patient’s electronic medical record. Photos can be viewed, added/updated, documented as declined, and removed from within Registration.
  • Potential Duplicate Records Window - (Handout) When creating a new patient MRN (Medical Record Number), the Potential Duplicate Records Window will pop up showing any patients with similar information. Closely review this information to determine if a potential duplicate patient may already exist. If there’s an existing match, then use the existing patient MRN rather than creating a duplicate.
  • Running the First Access Detail Report(s) - (Handout) - These reports are used by Compliance and HIM staff to audit UI CareLink users who access a patient’s chart by matching on required patient demographics.  There is a report specific to UIHC staff members and one that is specific to CCI sites.
