Case Management is Epic's application to support inpatient case management workflows, including utilization management and discharge planning. The goals of Case Management include the achievement of optimal health and access to care and appropriate utilization of resources, balanced with the patient's right to self-determination.

Registration Note: All individuals should be registered for Session #50777; Epic Case Management for Nursing (Online). Registered individuals will receive an email confirmation for their registration in which they will find a link to the online course.

Roles: Level of Care (LOC) & Utilization Management Nurses.

Content: Overview of Epic, Patient Lists, Chart Review, Results Review, Patient Reports. Specialized content specific to each role is to be provided within the department once the above training is complete.

Registration Note: Social Workers who will be working in the Psychiatric Outpatient Clinics should be registered for the Epic Ancillary course found HERE.

Roles: Social Workers & Nurse Navigators who perform post-discharge planning and service referrals.

Pre-requisite Content: Overview of Epic, Patient Lists, Patient Schedule, Patient Station & Open Chart, Chart Review, and Patient Reports.

In-Class Content: Login & Overview, Initiating a Destination Referral, Documenting an Assessment and Progress Note, Reviewing Referral Documentation, Completing an Agency Referral, Consults, Sending Supplemental Documentation, and Documenting a Telephone Call.

Class Length: Independent

Prerequisites: None

Registration: (No registration is required.) This course is online with additional content provided by the department. Successful completion of the online course (linked below) will generate a request to the Epic Training Team so that they can facilitate updated system access.
