Phoenix is Epic's Transplant application that focuses on the continuum of care from the initial evaluation to post-operative follow-ups. Support for patient surveillance, waitlist management, UNOS waitlist reconciliation and UNOS registry is included.

Resources

The following resources provide information on Transplant functionality.

General Resources

  • Auto Faxing External Labs - (Handout) Epic will send external lab orders to RightFax when they are released from treatment plans. Patients must have a Lab Preference set in the Demographics activity.

  • Auto Faxing Errors and Exceptions - (Handout) Common errors and exceptions to review when you handle processing of any external lab orders that fail to fax or must be handled in other ways such as printing, mailing, or sending to additional recipients.

  • Capturing Electronic Signatures (Transplant) Consent for Operations or Procedure Form - (Handout) How to capture electronic signatures for transplant Consent for Operation or Procedure Forms.

  • Episodes of Care - (Handout) An Episode is a function used in Epic to link together multiple encounters because they share a patient condition or treatment regimen such as pregnancy, transplant, dialysis, back pain or chemotherapy. This linking of encounters makes reviewing, reporting, and billing for them easier. An Episode Report can be found within the Chart Review activity for encounters that are linked in this way (such as Therapy or Treatment Plans).

  • UNOS/ABO Verification in OR (TXP901) - (Video: 7 min) Pre-Transplant Organ Verification must be properly documented to ensure the safety of organ transplant recipients and ensure that our program is compliant with regulatory mandates.

Bone Marrow Resources

  • BMT Referral Workflow - (Handout) When a new patient is referred to UIHC, the Referral Coordinator creates an Abstract Episode, collects information, then initiates the referral process.

Solid Organ Resources

  • Transplant Lab Encounters - (Handout) Draw Station schedulers can create One-Click encounters for releasing Transplant lab orders in Transplant Treatment Plan (Txp Lab Plan). Following this process makes the orders active and available for Phlebotomy to use when collecting specimens. Clinicians can release orders in Transplant Treatment Plan (Txp Lab Plan) activity.

  • UNOS TIEDI Forms - (Handout) UNOS registry form data is now imported to (and exported from) Epic. Some records will require review on the part of a Transplant Coordinator if there is missing data or patients/episodes that need to be manually associated.
