Population Health Performance Dashboards - (Handout) This document points out two dashboards Population Health – Provider Performance Dashboard and Population Health – Department Performance Dashboard. The population health provider performance dashboards are intended to provide an overview of your performance on selected measures as well as the incentives earned for addressing measures during a value-based member encounter with a Primary Care Provider.
