• Population Health Performance Dashboards - (Handout) This document points out two dashboards Population Health – Provider Performance Dashboard and Population Health – Department Performance Dashboard. The population health provider performance dashboards are intended to provide an overview of your performance on selected measures as well as the incentives earned for addressing measures during a value-based member encounter with a Primary Care Provider.
•  Value-Based Outreach WQ – (Handout) Patient Access staff on the Population Health team use value-based care gap workqueues to easily find and follow up on care gaps. This workflow is used to identify patients with open care gaps, perform outreach to patients, and set up appointments to close care gaps and meet goals for value-based contracts. This document outlines how to set up the value-based care gap workqueue, outreach to patients, and track outcomes of outreaches.
