MyChart
MyChart is an Epic application that allows patients to view their medical record and interact with their health care team. With MyChart, the patient can request an appointment, see test results, request a prescription renewal, message their health care team, and find helpful health tips and other trusted information. The MyChart website can be accessed from the UI Health Care website home page.

General Resources
- Overview of MyChart Patient Portal (MYC001) - (eLearning: 7.5 min) See how a patient uses MyChart and better understand how it interacts with Hyperspace.
Access/Activation
- Sending a MyChart Activation Message - (Handout) The MyChart Signup activity can be used to generate an activation code and email or text it to the patient. This is only available for patients age 12 and older.
- Completing the MyChart Signup Prompt during Check-In - (Handout) During the Check-In workflow, if a patient does not have an active MyChart account, the MyChart Signup prompt will display. Check-In staff will use the prompt to indicate the patient’s response.
- Grant Proxy Access (MYC005) - (eLearning: 4 min) Give a patient proxy access to the health information of another patient.
- MyChart - Two-Factor Authentication - (Handout) MyChart requires two-factor authentication which allows an extra layer of security to prevent unauthorized access to medical information.
- Proxy Access Through MyChart - (Handout) Parents/guardians can request proxy access for minors. When proxy access is granted, parents/guardians can view the minor’s medical records through MyChart.
- Requesting a MyChart Account electronically - (Handout) Patients can request creation of a MyChart account electronically.

Administrative Tools

- Cardiac Rehab Patient Care Companion - (Handout) - Cardiac Rehab Specialists will place an order to enroll patients in the Cardiac Rehab Care Companion, which allows patients to complete plan of care tasks such as reporting exercise data and bp stats through the MyChart mobile app (via Android or iOS device).
- Health Trends Report - (Handout) Health Trends is a component within the SnapShot report that may be used quickly review patient-entered data collected from MyChart Care Companion.
Information Sharing
- Authorizing Care Everywhere - (Handout) Organizations that also provide care for your patients might require authorization before allowing you to request patient information with Care Everywhere. Point-of-care authorizations allow patients to complete the required authorization, directly from their MyChart account.
- MyChart Information Sharing - (Website) A federal statute will soon require the immediate release of clinical notes and results to patients via MyChart. To meet this rule, UI Health Care is now releasing results in MyChart without delay.

When the completed questionnaire comes back, it will go directly to the responsible person who assigned the questionnaire to follow-up on the responses from patients.
