Medication History
Medication History is an activity that displays the patient's current medications including prescriptions and patient reported medications. The clinician is able to document the Last Dose,Taking status, and notes for the medication.

The following resources provide information on Medication History functionality.
- Add / Replace Preferred Pharmacy (and Send Orders to Multiple Pharmacies) - (Handout) Update the pharmacy (or pharmacies) that new prescriptions will be sent to both for the current encounter and to save time in future encounters.
- Discontinuing Medications - (Handout) Discontinuing medications that are no longer applicable from the Medication History section will update the patient’s current medication list and ensure that the After Visit Summary (AVS) is accurate.
- Documenting Medication Reconciliation - (Handout) Correctly and accurately updating Medication History is one of the single most important steps to improving medication safety. Medication reconciliation should only by performed by trained and approved staff as defined in UIHC Hospital policy CC.P.38
