The Medication Administration Record (MAR) is the activity in the patient’s chart where medications can be administered, reviewed, edited as well as tools to communicate with pharmacy relating to specific medications and administrations.

  • Adding a Due (Stopped) Time on Continuous Infusions - (Handout) To help nurses remember to stop continuous medication administrations on the MAR, Epic offers Due Actions in the MAR Administration tool. Nurses should add a Due (Stopped) time BEFORE acknowledging continuous medication orders with a new or modified End Time.

  • Advanced Medication Administration on the MAR (INP162) - (eLearning: 12 min) Document medication administrations for scheduled, overdue, missed, and held medications as well as override pulls using barcoding, the MAR, and the Brain.

  • Administer Medications in Rover - (Handout) The Rover application allows you to administer medications to your patients similar to the way you administer meds on the MAR in Hyperspace.

  • Document Multiple IVIG Vials Administration - (Handout) IVIG (IV immune globulin) may be dispensed in multiple vials to make up an ordered dose. There will be 2 orders on the MAR for an IVIG order. One being the full dose ordered and to be documented against first, by scanning all of the dispensed vials; and the MAR action of IVIG (scan ALL vials) would be utilized. The second order will include “infusion documentation” in the IVIG line name; and will be used to scan each vial in real time with hanging/infusing them; and the potential MAR actions of First Vial IVIG, Next Vial IVIG, and Last Vial IVIG correlating with which vial is being hung.

  • Drug Level Nursing Reminders - (Handout) After orders are placed for drug levels for vancomycin, aminoglycosides, immunosuppressants, or mood stabilizers/antiepileptics pharmacy will place a “lab draw nursing reminder” on the MAR for the appropriate due date and time. Following the lab collection, the nurse will also document on the MAR that it was collected.

  • Inpatient Nurse - Medication Administration - (Handout) Basic overview of the MAR activity as well as documentation of mediations, rescheduling a single due time, the MAR report, leaving MAR notes, etc.

  • Inpatient Nurse - Medication Administration Override Pulls - (Handout) If the order for a medication hasn't been verified by the pharmacy yet, that med doesn't appear on the patient's Automatic Dispensing Station ADS) profile. If the patient needs the med STAT and you can't wait for the pharmacy, you can pull the med from the ADS on override, link the med to the correct order, and administer the med.

  • Barcode Scanning for Administering Medications - (Handout) New SmartText Labels have been designed to help Pharmacy and Nursing provide the safest process for administering medications on the Medication Administration Record. It is therefore important that the correct barcodes are scanned when administering medications on the MAR.

  • MAR Patch Removal Documentation - (Handout) Medication patches (like lidocaine, nicotine, fentanyl, etc) have a unique workflow for documenting the removal of patches.

  • Multi-Vial Vaccine Ordering and Administration - (Handout) Multi-vial vaccines (such as PENTACEL, MENVEO, SHINGRIX) require special ordering, preparation and administration instructions.

  • Place a Hold on an Active Medication - (Handout) Providers can place holds on active scheduled medications from the Orders activity. Holds can be set to expire within a certain timeframe, at a certain time, or until manually unheld. This hold is visible to nursing staff on the MAR and also displays for the pharmacy staff. A Best Practice Advisory (BPA) reminds providers when a medication has been held for two or more administrations.
