Deficiency Tracking is part of Epic's Health Information Management system and used to create chart deficiencies, monitor deficiencies for completion, and take actions on outstanding deficiencies.

The following resources provide information on Deficiency Tracking functionality.

  • Closing an Open Clinic Encounter from In Basket - (Handout) Open Clinic Encounters may be viewed and signed (a.k.a. closed) from the My Open Charts In Basket folder. Note: Users other than the provider should NOT sign encounters unless authorized to do so.

  • Correcting Notes on the Wrong Encounter - (Handout) When a note has been attached to the wrong encounter, it can be corrected using one of three workflows: copy/paste into a new note, copy previous into a new note, or move the note.

  • Deficiency Tracking Workqueues - (Handout) Deficiency Tracking Workqueues allow Chart Control staff to monitor and update chart deficiencies from one place.

  • Reasons for an Open Encounter - (Handout) When a clinician has completed documentation and orders for a patient encounter, that encounter can be closed. At the time of closing an encounter, checks can run to verify that the clinician has entered all necessary information.

  • Viewing a Provider’s In Basket Deficiencies (Chart Control Staff) - (Handout) To assist in monitoring clinicians’ deficiencies, users may attach to a provider’s In Basket and review the provider’s Chart Reminders and My Open Charts folders.
